This is pertinent to students enrolled in a graduate and undergraduate programs whose status is absence without leave (AWOL). However, AWOL status does not apply to the Professional Teaching Certificate program. PTC students who failed to enroll for a specific term and would like to enroll for the next term just needs to send an email to the Program Chair (PC) signifying their intention of going back to the program. Sending the email of intent to enroll should be done a month before the start of the registration period.
- Accomplished Application Form for Readmission;
- Application fee payment receipt amounting to Php 225.00. See procedures for payment of fees.
How to apply for for Readmission:
Step 1: Mail or bring the accomplished form and payment receipt to the Office of the University Registrar (OUR) at:

or email scanned copies to [email protected] before the set deadline for the term (refer to the academic calendar for deadlines). Applications received beyond the deadline will be processed for the next term except those with the endorsement from the Faculty Office.
Step 2: Wait for an email from the OUR for the results. Results of the evaluation will be released after the evaluation of the Faculty of Study.
